Insurance Information

If you would like your visits to be billed through insurance, please select one of the following items when scheduling: 

New clients: "Using Insurance- Initial Visit"

Returning clients: "Using Insurance- Follow-up Visit"

We currently accept Cigna, Aetna, Anthem BCBS, United Healthcare, UMR and Medicare.

Note:  Medicare covers people with diabetes or chronic kidney disease stage 3-5 for RD visits.  Other conditions are not covered under Medicare.  Referral form below.

If you have a different plan, we can supply you a Superbill you will turn into your insurance for full or partial reimbursement of your visits as an "out of network" provider.

Please check with your plan to see if you are covered. Here is the information you may need to ask your plan:

  • Is my provider in-network?  If not, can I submit a superbill for possible reimbursement?

  • Am I covered for telehealth video/audio Medical Nutrition Therapy, codes 97802, 97803?
  • Diagnosis codes: ask if you are covered for the reason you’re being seen, like high cholesterol E78.5, type 2 diabetes E11.9, prediabetes R73.03 or PCOS E28.2. (More codes here)
    • Most plans cover code Z71.3 for dietary counseling as prevention. Often times with preventive counseling, there is no out-of-pocket cost.

Flex-spending
If you have a Flex-spending or Healthcare Savings account (HSA) through your employer we can accept that for payment of co-pays, deductibles and all private pay options.

Superbill form for reimbursement
We are happy to give you a “Superbill” to submit to your insurance plan for possible reimbursement. This can also be used as receipt for your flex-spending or HSA. Just ask.

Cancelation policy
24 hours advanced notice is required for ALL cancellations or to reschedule your appointment. If appointments or classes are not canceled or rescheduled in this manner, a cancellation charge of $25 will apply. Thank you for your cooperation with this policy.

Medicare
For people with Medicare, diabetes and chronic kidney disease are covered medical conditions to see a dietitian. If you have Medicare, please have a completed referral form before the first visit. Click Here.
